The Good Neighbour Scheme

This offers the kind of straightforward help a ‘good neighbour' might give. The Co-ordinator takes referrals from a wide range of sources Including District Nurses, G.P's, Social work Teams and various intermediate care schemes. The Co-ordinator also takes many referrals from relatives and potential service users.

The majority of tasks are carried out by our volunteers according to their skills and availability and they keep in close touch with the Co-ordinator. Wherever possible inappropriate referrals are passed on to relevant agencies.

EscortCare

This scheme is for patients who have had a full needs assessment and are going into residential care. Sessional workers take the patient and/or their carer to look at available places in the homes of their choice and return them to the hospital. Referrals come primarily from hospital Social Workers.

Hospital AfterCare Scheme

This scheme provides a wide range of practical help for patients following admission to or discharge from hospital. The three co-ordinators take referrals from hospital staff, particularly social work teams, physiotherapists and occupational therapists. The scheme operates at the Northern General Hospital, the Royal Hallamshire Hospital and Weston Park Hospital.

Accident and Emergency to Home Scheme

This service covers the A&E Department, Huntsman 1 and CMU at the Northern General Hospital and the Emergency Admission Unit and the Minor Injuries Unit at the Royal Hallamshire Hospital.

It is aimed at older people who are taken to the hospital with a minor injury and who are well enough to go home with some support. This service operates between 6am and 10pm,  seven days per week - including Bank Holidays. This scheme also takes referrals from the Hospital Assessment and Integrated Care Scheme.

Looking After the Whole Person

This was a 3 year project that provided a range of resources and training for staff working in residential homes, the project enabled older people living in long term care to shape the spiritual and emotional support that they received, using the resources available in the wider community. Already, a number of initiatives have evolved from this work, including two national events and a broad range of publicity and partnership working. This project acted as a bridge between churches, volunteers and residential homes to help make life in residential care as interesting and varied as possible. Whilst the project term has now ended the experience and information gained is still very influential in other areas of work.

Locally Based Schemes

SCCCC has also supported 2 satellite schemes called Care in Lowedges, Batemoor and Jordanthorpe and Care in Gleadless Valley. To view more information about these projects follow the link on the Home page. SCCCC also has a close working relationship with Care in Crosspool.

As a citywide charity, SCCCC continues to build relationships with Churches, other faith based organisations and local community projects as they work to support older people.  

Working with older people and their carers.

SCCCC has carried out a number of in-depth surveys of the views of older service users and their carers. These have been used by our partners in the statutory sector in the development of new services for older people.
